Add Problem Report

  1. Click Add Problem Report to add reports.  

     
  2. Provide accurate information in each of sections 1 through 5.
    • Section 1 Candidate Details: You can select any of the options listed below, including Registry Number and Candidate Name.  

       
    • Section 2 Associate additional Problem Reports: This will show if the same applicant has submitted applications for any other problem reports.
    • Section 3 Summary: Report Type, Status, Opened Date, Closed Date, and Select Description are all required fields to be filled out.  

       
    • Section 4 Complaint Details: Received Date By, Dept of Health, State Incident ID, and Entity Type are all required fields to be filled out.  

       
    • Section 5 Actions: After selecting Add Form, the Problem Report Action Form will show up. Fill out the required fields for Select Date, Action Type, and Comment, then click Save

    • Section 6 Description: Select Date, Disposition type, and Comment are all required fields to be filled out.  

       
  3. Click Submit, after completing the aforementioned areas.  

     
  4. A pop-up confirming the new Problem Report appears after successfully Saved Response.
